1. The first thing i do is fiddle with the brightness and contrast of the pic. As you can see, the pic is pretty dull (i took it on my camera phone - yes that is me). I set mine to this: brightness: -47 contrast: +25. After that flaten the image (what I do is create a new layer and then press CTRL-SHIFT-ALT-E). This will then come your "base layer". your picture should now look like this:

2. Next I add a new raster layer and flood fill it with a dark blue (i used #111641). I then set it to exclusion at 100% opacity. pretty simple, yea?

3. Add a new raster layer and flood fill it with a dark tan (i used #C7C8A2). Set it to multiply at 100% opacity.

4. It's looking a bit to pukey yellow green for my tastes so I duplicate the base layer and drag it to the top and set it to screen at 100% opacity.

5. Because I'm aiming for a blue-ish tint (the walls behind me are actually like this really orangy peach), I add a new raster layer and flood fill it with a bright blue (i used #75B4D4). Then set that layer to multiply at 60% opacity.

6. Now, that's a little too blue so we'll dull it down. Duplicate the base and drag it all the way to the top and then desaturate it (CTRL-SHIFT-U). Then, set it to screen at 60% opacity.

7. I suppose this is optional. But I wanted to soften my pic a bit. So first I created a new layer and flattened my image (you know the drill kids CTRL-SHIFT-ALT-E). Then I duplicated it and blured it with gaussian blur (FILTER>BLUE>GAUSSIAN BLUR). I can't remember what number i did it at but it was probably around 6-7. Then I duplicated the new base layer (The one we made at the beginning of this step) and simply changed the opacity to 70%.

8. Now, shrink your picture and crop it.

9. Make sure you have layer beneath your picture. We're going to do some more fixing and stuff. Fill the layer underneath the picture with a color from your pictures background (in my case #D5E6EB).

10. Now we will fix the background and the shoulder. First delete that little bar using the lasso tool (feather: 5px). Then, using the smudge tool, complete the shoulder. Your icon should now look like this.

11. Now, create a new later and flood fill with black and then set it to overlay. Now it has that cool contrasty look to it.

12. Next, create a new layer between the top layer and the actual picture. Using the box tool, create a rectangle box and using the paint bucket, fill it black. You might want to hide your overlay layer while you're doing this in case you want to move your box around.

13. Next, take some brushes, any type you like (i used some brushes from Hybrid-Genisis but whatever you want works) and put them around the border of the box. It should look a bit like this:

14. Rotate you icon 90% Right. Then, making sure that this font is on top of all the other layers, using #08C7F0, write whatever you want in your box. I worte 'Bittersweet' Caps Lock in Times New Roman PS MT. Below it, I add 5 X's. You can do whatever you want. Then, rotate it back to its original position.

15. Finally, use this to have that bright blue light at the bottom of your picture.

Set it to screen and you are all done. XD
